DAISY Team Award Nomination Form

The annual DAISY Team Award for extraordinary nurse-led teams recognizes that, while an idea to achieve better patient and family outcomes may start with one individual, it often takes an entire team to implement it successfully. The DAISY Team Award is designed to honor collaboration by two or more people, led by a nurse, who identifies and meets the patient and/or patient's family needs by going above and beyond the traditional role of nursing.

The DAISY Team Award complements the DAISY Awards regularly presented to individual nurses.

Please nominate a team led by a direct-care nurse that makes a difference in the life of a patient, a patient’s family, an employee or the community they serve. Thank you for taking the time to nominate an extraordinary nursing team for this award.
