Deborah Fernandez, a 55-year-old patient at Piedmont, suffers from chronic health conditions. Last June, she began working with Loretta Lane, a Piedmont outpatient care manager. And today they are meeting for the first time.

“When Loretta calls, it’s like a little business, a little personal, and then whatever else happens,” laughs Fernandez.
Outpatient care managers provide telephone support to patients between office visits, encourage them, reinforce health education, and serve as a liaison between the patient and the extended care team.
“If I need something from the doctor, like the diabetes pill I take ... I talk to Loretta, and I have it in a couple of hours,” Fernandez says.
A life-changing experience
Since working with Lane, Fernandez has lost over 50 pounds, stopped smoking and is able to walk moderate distances without the assistance of her scooter.
“I’m so proud of you, and all of the work that you’ve done,” Loretta says. “You are my biggest success story.”
“You’ve helped me,” Fernandez says. “And I feel much better.”
