Personalized Experience
With the guidance of your doctor, we’ll create personalized educational experiences and customized outreach, to support you in achieving your health goals. If you become a member of this program, we will assign you a dedicated nursing team that will assist with your health and care coordination. You may even get a device to keep at home that will collect your health data.
Who Should Enroll?
Patients who want to take a more active role in their health conditions and who want more support from their care team may be interested in this program. We started this program to help patients who are taking a proactive role in managing their diabetes, hypertension, lung disease, and other conditions.
How Does it Work?
Once you’re in the program, we will assign you a nurse Care Coordinator, who will call you regularly to help manage your health and well-being. Your Care Coordinator will help you get medication refills and coordinate transportation, community resources, and medical equipment. This can all be done over the phone.
You may even get a device to keep at home that will collect your health data. Your device will send your health data electronically and securely to your nursing team, allowing them to monitor your vital signs without you having to leave the comfort of your home.

Is this program for emergency services?
The Care Connect program does not provide emergency response and operates during normal working hours, Monday to Friday, 8:00 a.m. to 5 p.m. EST.
What if I join and then want to un-enroll?
You can un-enroll in this program at any time without penalty.
